Seasonal Influenza Vaccination School Outreach Programme

AISHK is taking part in the Department of Health's Seasonal Influenza Vaccination Outreach Programme (SIVSOP) for schools. Dedicare Medical Centre will come to our school on Saturday 24 October between 8.30am and 11.45am to provide influenza vaccinations to our students in the Auditorium.   

From17 August, parents can use the eHealth mobile app to submit an e-consent form for their children to receive seasonal influenza vaccinations (SIV) under the programme. You can use the below QR code to start filling out the e-consent forms for your children. 

We encourage you to take advantage of the e-consent form as it is simpler than using traditional paper forms. 

If you are unable to complete the e-consent, paper-based forms are available upon request. You can request form/s by completing this simple form Seasonal Influenza Vaccination Paper Form Request  – Fill out form. A paper form will then be sent home with your children.  

Please note the e-consent QR code will be invalid after Sunday 13-September and paper-based forms must be returned to the school no later than Friday 11-September.
